Responsibilities
- Perform thorough patient assessments, including vital signs, medical history review, and physiological evaluations to determine appropriate care needs.
- Manage the intake process by coordinating with healthcare professionals, reviewing medical records, and ensuring compliance with health regulation policies such as HIPAA.
- Collaborate with physicians, case managers, and other healthcare team members to develop individualized care plans tailored to each patient's condition.
- Utilize Electronic Health Records (EHR) systems efficiently for documentation, discharge planning, and medical records management.
- Conduct diagnostic evaluations and assist in managing patients across various settings including inpatient units, outpatient clinics, and post-acute care environments such as hospice or home health.
- Provide patient education on procedures, treatment plans, and discharge instructions to promote understanding and compliance.
- Support clinical case management activities by overseeing patient progress, facilitating communication among providers, and ensuring quality standards are met.
- Other duties as assigned by the DON or designee
Requirements
- Valid Registered Nurse (RN) license with current state certification.
- Familiarity with EMR/EHR systems and electronic health records management.
- Strong understanding of health regulation policies including HIPAA compliance and medical terminology.
- Excellent patient service skills with the ability to communicate effectively with diverse populations and collaborate with healthcare professionals.
- Ability to perform detailed patient assessments, manage complex cases such as behavioral health or geriatrics, and handle diagnostic evaluations efficiently.
- Experience managing patients in inpatient, outpatient, or post-acute settings such as hospice or home health is preferred.
