Position Summary
Pathways Treatment Center is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our outpatient substance use disorder treatment team. The LPN will play an essential role in supporting clients receiving Medication Assisted Treatment (MAT) and behavioral health services by administering medications, coordinating care with providers, monitoring treatment compliance, and ensuring clients receive timely medical services.
The ideal candidate is organized, detail-oriented, and comfortable working with individuals experiencing substance use and co-occurring mental health disorders.
ResponsibilitiesClinical Responsibilities
- Administer injectable medications, including Medication Assisted Treatment (MAT) injections such as Sublocade, Brixadi, and Vivitrol, in accordance with provider orders and nursing standards.
- Administer oral and other prescribed medications within the LPN scope of practice.
- Monitor clients before and after medication administration for adverse reactions or complications.
- Obtain and document vital signs and other clinical observations.
- Complete accurate medication documentation within the electronic medical record.
Medication Coordination
- Track medication refill dates and proactively coordinate prescription renewals with providers and pharmacies.
- Maintain a tracking system to ensure all injections are administered on or before their scheduled due dates.
- Maintain accurate medication inventory and ensure proper storage and handling of medications.
Provider Support
- Facilitate appointments between clients and the Psychiatric Mental Health Nurse Practitioner (PMHNP/APRN).
- Prepare clients for provider visits by obtaining vital signs, reviewing medications, and identifying clinical concerns.
- Communicate significant medical or behavioral changes to the APRN and clinical team.
- Assist providers during medical evaluations and procedures as needed.
Patient Care
- Provide compassionate nursing care to individuals with substance use disorders and co-occurring mental health conditions.
- Build positive therapeutic relationships while maintaining professional boundaries.
- Educate clients on medications, treatment expectations, side effects, and medication compliance.
- Respond appropriately to medical concerns and emergencies within the outpatient setting.
Collaboration & Documentation
- Work closely with therapists, counselors, case managers, and medical providers to coordinate client care.
- Maintain timely, accurate, and complete documentation in the electronic medical record (Sunwave).
- Ensure compliance with HIPAA, OSHA, New Jersey Department of Health regulations, Joint Commission standards, and Pathways Treatment Center policies.
Qualifications
- Current unrestricted New Jersey Licensed Practical Nurse (LPN) license.
- Current BLS/CPR certification.
- Minimum one year of nursing experience preferred.
- Experience in behavioral health, addiction medicine, psychiatry, or Medication Assisted Treatment (MAT) preferred.
- Ability to safely administer injectable medications.
- Strong organizational skills with the ability to manage medication schedules and follow-up appointments.
- Excellent communication and interpersonal skills.
- Ability to work effectively with individuals experiencing substance use disorders and co-occurring mental health conditions.
- Proficiency with electronic medical records is preferred.
Schedule
- Monday – Friday
- 9:00 AM – 3:00 PM
- No weekends or on-call responsibilities.
Pay: $32.00 - $35.00 per hour
