About APEX Hospice & Palliative Care
APEX Hospice & Palliative Care is seeking compassionate, experienced Registered Nurses to join our growing team as Hospice RN Case Managers. We currently have opportunities available in the North Side of Chicago, Oak Park, and McHenry/Lake County service areas.
As a Hospice RN Case Manager, you will provide skilled nursing care, coordinate interdisciplinary services, and support patients and families through every stage of the hospice journey. Responsibilities
· Manage and coordinate an assigned hospice patient caseload.
· Perform comprehensive nursing assessments and develop individualized plans of care.
· Provide skilled nursing visits in patients' homes and long-term care facilities.
· Collaborate with physicians, nurse practitioners, hospice aides, social workers, chaplains, and other interdisciplinary team members.
· Educate patients and families regarding disease progression, medications, symptom management, and end-of-life care.
· Coordinate admissions, recertifications, and care transitions.
· Complete accurate and timely documentation in the electronic medical record.
· Participate in interdisciplinary team meetings and on-call rotation as assigned.
Qualifications
· Current Illinois Registered Nurse (RN) license.
· Minimum one year of nursing experience required.
· Hospice, home health, palliative care, or case management experience preferred.
· Excellent assessment, communication, and organizational skills.
· Valid Illinois driver's license, reliable transportation, and current auto insurance.
