Overview
We are seeking a hands-on Manager/Director of Care Management Operations – to lead daily operations for a care management program.
This role oversees care management operations, staffing, workflows, provider coordination, referral management, quality standards, and team performance. The ideal candidate is an execution-focused leader who can manage people, improve processes, and ensure timely patient engagement in a fast-paced healthcare environment.
Key Responsibilities
Care Management Operations
- Oversee daily operations
- Ensure timely patient outreach, engagement, enrollment, documentation, and follow-up
- Monitor workflows and patient progression through care pathways
- Identify and resolve operational barriers and improve processes
Team Leadership
- Supervise Care Managers and support staff
- Set performance expectations for productivity, communication, and documentation
- Conduct coaching, check-ins, onboarding, and training
- Support performance management and accountability
Staffing & Capacity Planning
- Manage staffing coverage based on patient volume and referral activity
- Forecast staffing needs and adjust coverage to support growth and demand
Referral & Patient Flow
- Track referrals from SNFs, Home Health Agencies, and partners
- Ensure timely patient outreach and enrollment after discharge
- Identify bottlenecks in referral conversion and care engagement
Provider & Partner Coordination
- Serve as operational liaison between Care Managers, providers, SNFs, and Home Health partners
- Support scheduling, coordination, and escalation needs
- Maintain clear communication across internal and external teams
Quality & Compliance
- Oversee QA reviews of documentation and workflows
- Ensure adherence to care management standards and escalation protocols
- Support corrective action plans and process improvements
Process Improvement & Training
- Develop workflows, training materials, and job aids
- Lead training on new processes and operational updates
- Continuously improve efficiency and consistency across operations
Reporting & Metrics
- Track operational performance, staffing, referral flow, and patient engagement
- Provide updates to leadership using tools such as Monday.com
- Identify trends and drive operational improvements
Qualifications
Required
- 3–5+ years healthcare operations, care management, home health, SNF, or post-acute experience
- Experience supervising frontline staff
- Strong organizational and operational skills
- Excellent communication and follow-through
- Proficiency in Microsoft Office and project management tools
- Ability to build workflows and training materials
- Strong problem-solving and escalation judgment
Preferred
- Home health, or SNF experience
- Experience with EHRs or care coordination tools
- Familiarity with referral workflows and patient transitions
- Experience with Monday.com or similar platforms
- QA or audit experience

Pay: $75,000.00 - $90,000.00 per year
