Position Description | Physical Therapist
The Physical Therapist is responsible for providing high-quality physical therapy services to clients in accordance with physician orders and in collaboration with the interdisciplinary care team. This role requires close coordination with the Clinical Manager and other ordered disciplines to ensure comprehensive, patient-centered care.
The Physical Therapist conducts thorough patient evaluations and develops individualized plans of care with measurable goals based on assessment findings and physician directives. The PT is also responsible for appropriately delegating aspects of patient care to team members, considering staff competencies, patient acuity, and the established plan of care.
This position requires a hardworking and committed professional with excellent communication skills, a positive and flexible approach, and the ability to mentor and educate team members effectively. Participation in team meetings, case conferences, and in-service education is expected as part of maintaining high standards of care and collaboration
Responsibilities
- Client Assessments to identify functional limitations, goals, and individualized treatment needs
- Develop and implement effective, evidence-based plans of care to improve mobility, strength, balance, and overall function
- Provide cost-effective treatment interventions aimed at reducing reliance on medications and avoiding unnecessary surgical procedures when appropriate
- Deliver personalized therapy services using a variety of treatment techniques tailored to each patient’s condition and goals
- Educate and motivate patients to actively participate in their treatment and achieve optimal outcomes
- Promote healthy lifestyles through therapeutic exercise, functional training, and wellness-focused programs
- Collaborate with physicians, the Clinical Manager, and other healthcare professionals to coordinate comprehensive patient care
- Monitor patient progress, evaluate treatment effectiveness, and adjust plans of care as needed
- Accurately document evaluations, treatments, progress, and outcomes in accordance with regulatory and agency standards
Skills & Qualifications
- Proven experience as a Physical Therapist, preferably in a home health or clinical setting
- Strong clinical knowledge and current understanding of physical therapy treatment practices
- Ability to effectively manage and engage patients with diverse needs and personalities
- Hardworking, dependable, and committed professional with a strong work ethic
- Excellent interpersonal and communication skills, both written and verbal
- Demonstrated professionalism, adaptability, and a positive approach to patient care
- Desire to continue learning and growing within the physical therapy profession
- Valid Physical Therapist license in good standing
Job Types: Full-time, PRN
Pay: $110,000.00 - $150,000.00 per year
