Job Overview
Cornerstone Health Centers, a functional medicine and integrative wellness practice in Flemington, NJ, is seeking a dedicated and compassionate Occupational Therapist to join our team. In this role, you'll assess, plan, and implement personalized therapy programs that help our patients improve daily functioning and quality of life — alongside the IV nutrition therapy, hyperbaric oxygen, and functional medicine services that define our integrative approach to care. You'll play a key role in facilitating recovery, promoting independence, and supporting patients through their rehabilitation journey, often as part of a broader, whole-person treatment plan.
Duties
- Conduct comprehensive evaluations of patients' physical, cognitive, and sensory abilities to develop tailored care plans
- Assist patients with Activities of Daily Living (ADLs) such as dressing, grooming, and mobility to foster independence
- Collaborate with our multidisciplinary team, including chiropractic, nursing, and functional medicine providers, to coordinate integrative care
- Lead discharge planning efforts, preparing patients for transition home or to other care settings
- Work with patients across the lifespan — adults, children, families, and geriatrics
- Maintain meticulous case documentation, including progress notes, treatment plans, and outcome reports, in compliance with healthcare regulations
- Educate patients and families on therapeutic exercises and adaptive techniques to support ongoing recovery
- Apply physiology, medical terminology, and anatomy expertise to inform effective, individualized treatment approaches
Experience
- Experience in inpatient settings (ICU, acute care) is a plus
- Demonstrated background working with patients across age groups — pediatrics through geriatrics — preferably in outpatient or clinic settings
- Experience in early intervention or school-based therapy is a strong asset
- Familiarity with occupational health practices and home health services is beneficial
- Strong case management skills, with a focus on discharge planning and care coordination
- Solid understanding of medical terminology and anatomy for precise assessment and intervention planning
- Interest in or familiarity with integrative/functional medicine settings is a plus, though not required
