Medicare RPM & CCM Clinical Team
Physicians Medical Director Nurse Practitioners
Remote | Part-Time / Flexible | Medicare Care Management
We are launching a technology-enabled Medicare Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) program and are building our clinical leadership and rendering-practitioner team.
We are seeking experienced, Medicare-enrolled MDs, DOs, and Nurse Practitioners interested in structured remote care with defined patient panels, strong clinical support, established protocols, and predictable time commitments.
We are recruiting for several roles:
1. Rendering Physicians — MD/DO
Serve as the Medicare billing practitioner for an assigned patient panel and provide clinical oversight of RPM and CCM services.
2. Medical Director / Physician Leadership — MD/DO
Provide senior clinical governance, approve clinical protocols, oversee quality and escalation standards, and support the clinical infrastructure of the program.
For selected professional entities, we are also interested in physicians who may be qualified and willing to participate in physician ownership and governance of a professional corporation or professional association where required or permitted by applicable state law. Any such arrangement will be separately structured, documented, and reviewed for compliance with applicable corporate-practice-of-medicine and professional-entity requirements.
This is intended to be a substantive physician governance role—not nominal ownership.
3. Nurse Practitioners / Advanced Practice Clinicians
Manage assigned Medicare patient panels, conduct appropriate clinical evaluations, establish and update care plans, review RPM escalations, and oversee delegated clinical workflows within applicable scope-of-practice requirements.
What You’ll Do
Depending on role and scope:
- Conduct initiating and follow-up evaluation and management visits
- Determine patient eligibility and medical necessity for RPM and/or CCM
- Establish, review, and update comprehensive care plans
- Manage a defined Medicare patient panel
- Provide appropriate supervision of clinical staff performing remote monitoring and care-management activities
- Review escalated blood pressure, glucose, weight, pulse oximetry, and other physiologic data
- Direct clinical interventions when thresholds or symptoms require escalation
- Approve standing orders, clinical pathways, and delegated protocols
- Coordinate care with patients’ other treating providers when appropriate
- Review documentation for clinical accuracy and billing support
- Participate in quality, compliance, and utilization review
- Support a highly structured, technology-enabled remote-care model
Required Qualifications
Physicians — MD/DO
- MD or DO with active, unrestricted medical license
- Active Medicare enrollment and current PECOS record
- Willingness to reassign Medicare benefits to our enrolled professional practice through the applicable Medicare/PECOS enrollment process
- Comfortable providing remote care and supervising delegated clinical workflows
- Good professional and regulatory standing
- Appropriate malpractice coverage or ability to obtain required coverage
- Strong documentation and clinical decision-making skills
Nurse Practitioners
- Active, unrestricted NP/APRN license
- Active Medicare enrollment strongly preferred
- Current PECOS record strongly preferred
- Ability to practice independently or under required collaborative arrangements based on applicable state law
- Comfortable managing chronic disease patients remotely
- Strong documentation and escalation-management skills
- Appropriate malpractice coverage or ability to obtain required coverage
Strongly Preferred
- Multi-state licensure
- Prior RPM or CCM experience
- Medicare population experience

- Familiarity with Medicare care-management documentation and compliance
- Experience supervising RNs, LPNs, MAs, care coordinators, or remote clinical teams

The Opportunity
This is not a traditional high-volume telemedicine position.
We are building a structured clinical operating model in which technology and trained clinical staff manage much of the routine monitoring, documentation, outreach, and workflow while the rendering practitioner remains responsible for the clinical decisions that require practitioner judgment.

We are especially interested in clinicians who can begin quickly and who already have the licensing and Medicare enrollment infrastructure necessary to support a growing multi-state Medicare population.
Pay: $27.91 - $33.61 per hour
