We are seeking an experienced, compassionate, and organized Clinical Supervisor to oversee the clinical operations and field staff of our growing home health agency. This position is responsible for providing clinical oversight, support, and guidance to our Certified Nursing Assistants (CNAs), Home Health Aides (HHAs), Personal Care Aides (PCAs), and Direct Support Professionals (DSPs), ensuring they deliver safe, high-quality, patient-centered care in the home.
The ideal candidate is a Licensed Practical Nurse (LPN) or Registered Nurse (RN) with home health experience who enjoys mentoring others, maintaining clinical excellence, and ensuring compliance with agency policies and state regulations. Previous management or supervisory experience is strongly preferred.
Responsibilities
- Provide clinical oversight and supervision to CNAs, HHAs, PCAs, and DSPs providing care in patients' homes.
- Conduct supervisory visits, patient assessments, and reassessments in accordance with agency policies and state regulations.
- Ensure caregivers are delivering quality care while following individualized care plans.
- Serve as a clinical resource for caregivers by answering questions, providing coaching, and offering guidance on patient care concerns.
- Assist with orientation, training, competency evaluations, and ongoing education for direct care staff.
- Monitor documentation for accuracy, completeness, and regulatory compliance.
- Collaborate with physicians, case managers, families, and community partners to coordinate patient care.
- Participate in incident investigations, corrective action planning, and quality improvement initiatives.
- Ensure compliance with HIPAA, state licensing requirements, infection control standards, and agency policies.
- Support scheduling and staffing teams by helping address clinical concerns and caregiver assignments when needed.
- Foster a positive, accountable, and supportive culture focused on exceptional patient care and employee development.
- Participate in on-call clinical support as needed.
Qualifications
- Active Licensed Practical Nurse (LPN) or Registered Nurse (RN) license in good standing.
- Minimum of 2–3 years of home health or home care experience required.
- Previous supervisory, management, or leadership experience is strongly preferred.
- Experience supervising or supporting field staff, including CNAs, HHAs, PCAs, or DSPs, is highly desirable.
- Strong understanding of home health regulations, documentation requirements, and best practices.
- Excellent communication, leadership, coaching, and conflict-resolution skills.
- Ability to work independently while managing multiple priorities.
- Proficiency with electronic medical record (EMR) systems and Microsoft Office.
- Valid driver's license, reliable transportation, and the ability to travel to patient homes for supervisory visits.
Pay: $31.00 - $36.00 per hour
