Legacy Hospice and Palliative Care, located in Chino, CA, is currently looking for compassionate and caring LVN's to assist with PER- DIEM Continuous Care assignments.
We are a hospice and palliative care agency seeking LVN's to do per-diem shift care to address patient and family concerns, and work with their team to ensure communication of important information and care plan changes. Must have working knowledge of HospiceMD. Come be a part of our TEAM!!!
