The Workforce Development & Transitional Support Coordinator plays a critical role in preparing
youth for life beyond foster care. This position is designed for a visionary, hands-on leader who
understands the unique challenges faced by youth aging out of care and is passionate about
equipping them with the tools, relationships, and opportunities needed to thrive independently.
You will lead the design and implementation of workforce readiness programs, life skills training,
and transitional support services for youth in Supervised Independent Living (SIL) and extended
care placements. This includes building strategic partnerships with employers, educators, and
service providers, while maintaining compliance with state regulations and trauma-informed best
practices.
Key Responsibilities
Youth Empowerment & Case Coordination
- Develop individualized transition plans that include education, employment, housing, and
wellness goals
- Facilitate life skills workshops covering budgeting, time management, job readiness, and
self-advocacy
- Provide one-on-one coaching and mentorship to youth navigating adulthood
- Coordinate referrals to community resources, vocational programs, and mental health
services
Program Development & Strategic Partnerships
- Build and maintain relationships with local employers, colleges, training centers, and
housing providers
- Organize job fairs, career panels, and internship opportunities tailored to youth interests
and goals
- Collaborate with internal teams to align transitional support with placement stability and
permanency planning
- Track program outcomes and contribute to grant reporting and strategic planning
Compliance & Documentation
- Maintain accurate and timely documentation in accordance with DFPS and agency
standards
- Prepare for audits, licensing visits, and contract renewals with thorough records and
reporting
- Ensure all youth interactions are trauma-informed, culturally responsive, and
confidentiality-protected
Community Engagement & Advocacy
- Represent Houston Strong at community events, youth summits, and stakeholder
meetings
- Advocate for policy improvements and funding that support youth aging out of care
- Elevate youth voices through storytelling, feedback loops, and leadership development
Qualifications
- Bachelor’s degree in Social Work, Education, Human Services, or related field (Master’s
preferred)
- 2–4 years of experience in youth development, workforce programming, or case
management
- Deep understanding of trauma-informed care, adolescent development, and systems
navigation
- Strong communication, organization, and relationship-building skills
- Proficiency in Microsoft Office, Google Workspace, and case management systems
- Bilingual (Spanish/English) a plus
