Job Details -
- Must have recent 2 years of Patient Care Coordinator experience.
- Licensing: Active California Registered Nurse (RN) license.
- Certifications: Current Basic Life Support (BLS) certification.
- Experience: 1 to 3+ years of clinical nursing, preferably in home health, acute care (ER/ICU), or case management.
- Skills: Proficiency with EHR systems, strong clinical problem-solving abilities, and robust knowledge of local regulatory requirements
