Locally owned growing hospice looking for an experienced LMSW. Must have hospice experience and be dedicated to patient care.
Must be able to provide grief therapy.
Knowledge in developing and updating care plans is a must.
Proficient in computer systems required.
PASSR form completion a requirement
GA Medicaid application knowledge is also required.
Previous hospice experience is required (1yr minimum)
