Senior Care Solutions is seeking a compassionate, dependable, and patient-focused Home Health Certified Nursing Assistant (CNA) to join our growing clinical team. The CNA provides hands-on personal care services to patients in their homes under the supervision of a Registered Nurse and in accordance with the patient’s individualized plan of care.
This role is ideal for someone who thrives in an independent environment, values meaningful one-on-one patient interaction, and is passionate about delivering high-quality home health care.
Provide personal care services including bathing, grooming, dressing, toileting, and oral hygiene
Assist with mobility, transfers, ambulation, and range of motion exercises
Monitor and report changes in patient condition to supervising nurse
Assist with light housekeeping related to patient care
Maintain accurate, timely documentation of visits in EMR
Follow infection control, safety, and HIPAA guidelines
Active Florida CNA License in good standing
Current CPR certification
Valid driver’s license and reliable transportation
Auto insurance (meeting state minimum requirements)
Ability to pass Level 2 background screening (AHCA compliant)
Previous home health experience preferred but not required
Experience with Wellsky/Kinnser preferred but not required
Field-based position in patient homes
