Position Title: Consultant Pharmacist
Job Type: Full-Time, Monday through Friday. This is a hybrid position (50% traveling to various facilities and 50% remote).
Location: Denver, CO
Pay: $130,000- $137,000 per year. Pay is dependent on applicant’s relevant experience.
Duties
The Consultant Pharmacist will collaborate with facility leaders, staff, and the Clinical Resource Team to oversee all medication-related systems and conduct medication regimen reviews for residents within Endura long term care facilities in Colorado. You will assist the facility to identify, evaluate, and address/resolve medication-related concerns and issues that affect resident care, medical care, quality of life, and/or regulatory compliance. This position will perform duties below by working in and remotely for various long-term care facilities:
- Ensure facilities remain compliant with federal and state regulations.
- Complete clinical chart reviews as required by federal and state regulation.
- Ensure facility compliance with medication-related policies and procedures.
- Maintain knowledge of pharmaceutical care practices for long term care residents.
- Own issue resolution and communicate early warning signs of potential issues.
- Evaluate physician medication orders for appropriateness of drug, dosage, potential interactions, route of administration and stability. Make recommendations for therapy changes to the physician and facility staff as appropriate.
- Utilize RxPertise Consulting Software to perform Medication Regimen Reviews (MRR), as required by regulation at least monthly and report any irregularities to the Director of Nursing, Executive Director, Medical Director, and the attending physicians with a written report of findings and recommendations.
- Spend an appropriate amount of time on each resident’s review as defined by their clinical needs.
- Assist facilities with medication cost containment, including pharmacy therapeutic interchange programs and reduction of polypharmacy.
- Complete all required reports.
- Work cooperatively as a member of a team.
- Prepare reports and participate in quality assurance and psychotropic committee meetings at each facility.
- Support facilities during CPDHE surveys and reduce the potential for medication-related citations.
- Observe medication administration by nursing staff, monitor for medication errors, and provide written report of findings. Educate nursing staff on proper medication administration.
- Conduct medication storage audits to ensure proper storage, labeling and expiration dating of medications. Evaluate controlled substance medication inventory counts and proper storage.
- Provide education/in-services to nursing staff and other team members regarding medication-related subjects.
- Consult facility on proper medication disposal, including hazardous and controlled substance medications.
Requirements:
- Minimum bachelor’s degree from an accredited school of Pharmacy
- Current Colorado license required
- Clinical experience in acute or post-acute/long-term care
- Ability to learn and understand state and federal regulations for long-term care
- Detail and goal oriented, team player with focus on positive culture
