A fast-growing long-term care organization is seeking a compassionate and dedicated Nurse Practitioner to join its team at a nursing home in Briarcliff Manor, NY. This is an excellent opportunity for an experienced provider who is passionate about delivering high-quality primary care to a geriatric patient population in a supportive and collaborative environment.
Responsibilities:

Provide comprehensive primary care services to nursing home residents

Perform new admission evaluations and ongoing follow-up visits

Monitor, assess, and manage acute and chronic medical conditions

Collaborate with interdisciplinary care teams to ensure optimal patient outcomes

Document patient encounters thoroughly and accurately in the medical record

Support continuity of care and contribute to overall quality of care within the facility
Requirements:

Active Nurse Practitioner license in the state of New York

Active DEA license

Minimum 6 months of experience working as a Nurse Practitioner

Prior nursing home or long-term care experience strongly preferred
Compensation & Benefits:
Highly competitive salary: $150,000–$155,000 annually
