Duties
- Assist patients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support using patient transfer methods such as Hoyer lifts when necessary
- Monitor patient observations, vital signs, and overall health status; document medical information accurately using Electronic Health Record (EHR) systems or medical records
- Provide specialized care for individuals with dementia, Alzheimer’s disease, or other memory impairments in memory or hospice care settings
- Administer medications and perform catheter care following home healthcare regulations and medical terminology standards
- Support patients during end-of-life care with compassion and respect while maintaining patient confidentiality in accordance with HIPAA guidelines
- Assist with meal preparation, feeding, laundry, cleaning, and general household tasks to promote a safe and hygienic environment
- Utilize patient transfer techniques safely during patient movement or transfers to prevent injury for both patients and staff
Qualifications
- Prior experience working in assisted living facilities, nursing homes, or home healthcare environments is highly preferred
- Knowledge of medical terminology, vital signs measurement, medication administration, and basic life support (CPR/First Aid) certifications required or obtainable upon hire
- Demonstrated experience working with geriatric patients or individuals with disabilities such as developmental delays or dementia is desirable
- Ability to lift heavy objects safely and perform physical tasks involved in patient transfer methods such as Hoyer lifts or other mobility aids
- Strong communication skills for effective client interaction and documentation of medical records
- Familiarity with EMR/EHR systems and home health care regulations to ensure compliance and accurate recordkeeping
- Compassionate caregiving attitude with a focus on patient personal care assistance and client communication
