We have four openings at four of our medical offices providing health care to the local community. We are looking for Care Navigators that can work in a medical office practice. Candidate must be able to multitask and have a good clinical and clerical background. This is a full-time position with full benefits, or part-time flexible hours. POSITION PURPOSE
Responsible for improving patient’s level of engagement, overall experience with Primary Care services and ensuring patient experience for services are consistent with the goals of the organization. The Care Navigator is responsible for ensuring effective coordination of all patient appointments as well as subsequent coordination of patient care. The role of the Care Navigator is crucial to patient engagement and establishing consistent care with Primary Care services for both medical and dental services.
Position Qualifications
Education:
Associates degree will accept, Bachelor’s degree preferred in health administration, Health Services Management, Health Science, Care Navigation or related degrees and medical background is preferred.
Experience/Specialized Skills/Aptitudes
Care Navigator must be a caring and effective communicator with patients and various healthcare professionals. Knowledge of health care delivery and skills related to customer/patient experience performance are preferred.
