The BHT provides temporary care and supervision for an individual who cannot provide for all of his/her needs. The BHT provides services as outlined by the individual’s IDT. These services may include supervision, monitoring, and if indicted in the IPP may provide implementation of a behavior support plan, implementation of medical plans of care and habilitation training.
QUALIFICATIONS:
1. Must be at least 18 years of age.
2. Must have successfully completed a CIB Background check.
3. Must successfully complete the established agency training requirements.
4. Must maintain favorable performance evaluations.
CUSTOMER SERVICE
A. Accompanies/transports consumer on community outings and to appointments when necessary.
B. Assists consumer who requires help in any toileting activity.
C. Responsible for lifting consumer and changing diapers for the consumers who require this assistance.
D. Responsible for assisting consumer in maintaining personal hygiene and appearance.
E. Ensures the care, safety, welfare, and security of the consumer.
QUALITY IMPROVEMENT/UTILIZATION MANAGEMENT
A. Responsible for reporting suspected/alleged abuse and/or neglect to the proper authorities in a timely manner.
B. Attends all trainings as required by state licensure and other governing bodies.
C. Accurately completes and submits monthly progress notes and DD-11 by the assigned due date.
