About the Role:
As a Physical Therapist at West Park Rehabilitation and Nursing Center, you will be responsible for:
Designing and implementing comprehensive rehabilitation plans for our residents, focusing on improving functional abilities and overall well-being
Conducting thorough evaluations and assessments to identify physical therapy needs
Developing and implementing individualized treatment plans to address resident goals and outcomes
Collaborating with multidisciplinary teams to ensure residents receive coordinated, patient-centered care
Providing education and training to residents, families, and caregivers on physical therapy techniques and exercises
Documenting treatment plans, progress notes, and outcomes in a timely and accurate manner
