Responsibilities
- Conduct comprehensive evaluations to determine patients’ functional abilities and develop customized care plans.
- Assist patients with Activities of Daily Living (ADLs) such as dressing, grooming, feeding, and mobility to promote independence.
- Collaborate with multidisciplinary teams to coordinate discharge planning and ensure seamless transitions across care settings.
- Provide therapy interventions for diverse populations including pediatrics, geriatrics, individuals with autism, and those in acute or post-acute care.
- Implement therapeutic techniques in various environments such as inpatient units, skilled nursing facilities, outpatient clinics, schools, and home health settings.
- Maintain detailed documentation of patient progress, treatment plans, and case management activities following medical terminology and anatomy knowledge standards.
- Educate patients and families on therapy goals, adaptive equipment use, and strategies to enhance daily functioning.
Experience
- Proven experience working with individuals with disabilities across multiple settings including inpatient hospital units, outpatient clinics, schools, or skilled nursing facilities.
- Demonstrated ability to assist with ADLs and develop effective discharge plans tailored to each patient’s needs.
- Background in early intervention programs or pediatric outpatient services involving speech therapy integration is a plus.
- Familiarity with physiology principles, anatomy knowledge, and medical terminology essential for accurate assessments and treatment planning.
- Prior involvement in case management activities within post-acute or home health care settings preferred.

Pay: $70.00 - $80.00 per hour
