Overview
In this role as a Patient Care Coordinator supporting 1 For All Home Healthcare patients, you will be instrumental in delivering a seamless care experience. We are seeking a dedicated and compassionate Patient Care Coordinator to join our home healthcare team. You will also serve as a key contributor to our patient concierge experience. This position requires strong communication skills, attention to detail, and the ability to work effectively in a team environment.
Duties
- Maintain accurate medical records using Electronic Health Record (EHR) systems such as AxisCare.
- Ensure compliance with HIPAA regulations while handling sensitive patient information.
- Provide clerical support including scheduling appointments, make schedule changes, directing phone calls to the correct location, and running required monthly checks.
- Collaborate with our Registered Nurse to develop and implement patient care plans.
- Exhibit excellent customer service skills while interacting with patients and their families.
- Other duties as assigned
Requirements
- Answering phones by the second ring. Acting as an operator and directing calls.
- Performing professional, kind, and caring customer service
- Consistently upbeat and helpful to clients and staff
- Help with scheduling
- Billing and data entry work in an electronic health record database with accuracy.
- Working independently with minimum supervision. Able to follow established procedures.
Qualifications:
- High School Diploma or equivalent
- Communication skills – must be able to relate well to office team, caregivers, and patients
- Ability to multitask, organizational detail, ability to meet deadlines, work with little to no supervision
- As a member of the team, you must possess efficient time management
- HCBS waiver knowledge and Microsoft suite experience is a plus, but not required.
Conditions of Employment:
- This position will be an in-person position with potential to be hybrid
- Standard background checks - OIG, CIB, CAPS, Sex Offender
- Current/Valid and unrestricted US Driver's License
Additional Information
- Job Status: Full Time 40 hours/week
- Job Schedule: Monday - Friday 8:00am - 4:30pm. Occasional weekend handling phones.
- $20-$30 an hour based on experience, cognitive test results, and successful interviews
Job Type: Full-time
Pay: $20.00 - $25.00 per hour
