Mother Cabrini Health Foundation: Mobile Clinic Coordinator
Job Description:
The MCHF Wellness on Wheels Program Coordinator is housed within St. Bonaventure University's DePerro School of Health Professions (DSHP). The Coordinator serves as the primary operational lead for the Wellness on Wheels Mobile Health Program and works collaboratively with the DSHP Dean and faculty, Community Advisory Committee, healthcare providers, community organizations, public health agencies, and clinical partners.
Within this role, the Coordinator is responsible for planning, coordinating, implementing, and evaluating mobile health clinic activities designed to improve healthcare access and health outcomes in rural and underserved communities throughout Western New York. The Coordinator will organize planning meetings, coordinate community outreach initiatives, manage mobile clinic scheduling and logistics, support student workforce development, facilitate patient engagement and referral activities, collect and report program data, and assist with quality improvement efforts.
The Coordinator will represent the DSHP and work closely with academic Program Directors and faculty to identify and coordinate undergraduate and graduate student participation in mobile clinic activities. The Coordinator will facilitate interprofessional educational experiences involving students from multiple health professions programs while ensuring compliance with clinical, regulatory, university, and grant-related requirements.
The Coordinator will also assist in developing and maintaining community partnerships, identifying community health needs, supporting preventive health and wellness initiatives, facilitating access to healthcare resources and referral services, and preparing reports related to program outcomes and grant deliverables.
This staff position requires a highly motivated, organized, and community-oriented individual who is committed to improving health and wellness through innovative community-based programming. The position reports directly to the Dean of the DePerro School of Health Professions and requires strong leadership, communication, and collaborative skills.
This is a grant-funded position with funding currently secured through 2029. Continuation of the position beyond that date is contingent upon the availability of additional grant funding.
EXAMPLE OF DUTIES
Coordinate and manage Program related research components, IRB, mobile clinic operations, including scheduling, logistics, planning meetings, site coordination, and community-based service delivery activities.

Develop and maintain relationships with healthcare providers, community organizations, public health agencies, schools, faith-based organizations, and other community partners.

Work with the DSHP Dean, Program Directors, and faculty to identify, coordinate, and support student participation in mobile clinic activities and interprofessional education experiences.

Recruit, train, schedule, and supervise student interns and volunteers participating in mobile health, outreach, and wellness activities.

Coordinate community outreach, health promotion, preventive health screening, patient engagement, and referral activities.

Collect, manage, and analyze program data; prepare reports related to program outcomes, quality improvement, and grant requirements.
