As our Registered Nurse, you will work with our Behavioral Health Team providing nursing assessment and case management. You will support complex care for patients, engage in client advocacy and contribute to the plan of care that helps clients to reach healthy outcomes.

What You Get to Do:
- You will assist providers with direct patient care and case coordination.
- You will promote awareness of our services and support entry into care, including services for substance use treatment.
- You will provide nursing support outside of the clinic in designated collaborating organizations such as Home Health Care, TGTHR, All Roads, and Safehouse.
- You will triage patients and evaluate immediate treatment.
- You get to do Co-visits where you will evaluate patients, in collaboration with the provider, and treat patients with acute needs to help create access for patients.
Compensation: Approximately $34.70 - $43.95 per hour. All individual pay rates are calculated based on the candidate's experience and internal equity.
What We Need for this Job:

- Strong initiative to assist your team.
- Bilingual in English and Spanish desired.
