POSITION SUMMARY
AHNS is seeking a caring, credentialed, and driven Licensed Practical Nurse to serve as a Care Coordinator for our growing client base in Novi, MI office. This is a hybrid coordination role: you will assess prospective and existing clients, build individualized care plans, train and mentor Caregivers, and act as the trusted point of contact between clients, families, caregivers, and referral partners. You will also play an active role in representing AHNS in the community and supporting the agency's growth.
The ideal candidate blends nursing judgment with a consultative, relationship-first approach, someone who can walk into a client's home, put a worried family at ease, build a plan that actually fits their life, and then make sure that plan is executed with excellence day after day.
KEY RESPONSIBILITIES
Client Assessment & Care Planning
- Conduct in-home consultations and clinical assessments for prospective clients, using a consultative approach to identify needs and recommend appropriate services.
- Develop and implement individualized care plans aligned with each client's health goals, preferences, and home environment.
- Perform ongoing reassessments (minimum quarterly, or sooner as conditions change) and update care plans accordingly.
- Own the assessment-to-start conversion process, targeting at least a 90% conversion rate from initial consultation to service start.
Care Coordination & Quality Assurance
- Serve as the primary point of contact connecting clients, families, caregivers, and healthcare professionals.
- Coordinate caregiver schedules and services to ensure continuous, well-matched care delivery.
- Conduct regular quality-assurance visits and follow-up calls to confirm client satisfaction and catch concerns early.
- Respond to incoming inquiry calls, answer questions about services, and schedule assessment appointments.
- Maintain accurate, timely records in the client management/EHR portal, including assessments, care plans, and progress notes.
Caregiver Training & Support
- Design and lead training sessions for Caregivers and CNAs covering care protocols, safety procedures, and client-specific needs.
- Mentor caregivers on the ground, monitor performance against care standards, and ensure they have the resources and information needed to succeed.
- Champion caregiver concerns and feedback as a bridge between the field team and agency leadership.
Business Development & Community Engagement
- Actively participate in marketing efforts and represent AHNS at community events, health fairs, and networking meetings.
- Build and maintain relationships with healthcare professionals, hospitals, and community organizations that generate referrals.
- Collaborate with the marketing team on materials and campaigns that support new client acquisition.
- Manage incoming referrals to ensure timely follow-up and appropriate client placement.
Compliance & Documentation
- Ensure all client and caregiver documentation complies with Michigan home care regulations, Medicaid Home Help Program requirements, and AHNS policy.
- Maintain strict confidentiality of client health information in accordance with HIPAA.
QUALIFICATIONS
Required
- Active LPN license in the State of Michigan.
- Minimum one year of experience in home care, health care, or a senior-services setting (an equivalent combination of education and experience will be considered).
- Valid driver's license, reliable transportation, and willingness to travel throughout the Metro Detroit service area.
Preferred
- Prior experience training or supervising caregivers, CNAs, or home health aides.
- Familiarity with Michigan Medicaid Home Help Program or MI Choice Waiver documentation standards.
- Experience with home care scheduling/EHR platforms (e.g., WellSky, AxisCare, or similar).
KNOWLEDGE, SKILLS & ABILITIES
- Excellent oral and written communication skills, including the ability to listen effectively and de-escalate concerns with empathy.
- Strong organizational skills with high attention to detail for scheduling, documentation, and compliance.
- Comfortable and patient on the phone, with genuine warmth in client- and family-facing interactions.
- Proficient in Microsoft Excel and Word; able to quickly learn agency software systems.
- Ability to work both independently and collaboratively as part of a multidisciplinary team.
- Sound judgment, discretion, and integrity consistent with office standards.
- Flexibility to provide secondary on-call coverage during evenings or weekends.

Compensation & Schedule
Compensation is competitive and commensurate with experience and licensure. This is a full-time, non-exempt position based out of our Novi, MI office with regular travel throughout our Oakland, Macomb, and Wayne County service area. Some evening or weekend availability is required for on-call coverage.
