Hours: Monday - Friday 8am-5pm.
Required Education
- High School Diploma or equivalent
Preferred Education
- Associate's or Bachelor's degree in Healthcare Administration, Social Work, Psychology, Human Services, Nursing, or related field

Care Management Associate Overview
- Serve as the operational and administrative support partner for the OhioRISE Care Coordination department
- Act as the first point of contact for members, providers, guardians, and community partners
- Process referrals and verify Medicaid eligibility
- Complete Health Risk Assessments
- Respond to inbound inquiries and support the care management queue
- Maintain accurate documentation within electronic record systems
- Support care coordination activities while maintaining HIPAA compliance
- Deliver compassionate customer service to vulnerable populations
Job Duties
- Answer inbound care management inquiries
- Process OhioRISE referrals
- Verify Medicaid eligibility
- Complete Health Risk Assessments
- Document interactions in electronic systems
- Respond to member, guardian, and provider questions
- Support care coordinators with administrative tasks
