Responsibilities:
Provide direct care to residents, including assisting with daily living activities such as bathing, dressing, and grooming
Monitor vital signs and report any changes to the healthcare team
Assist with mobility and transfers as needed
Keep residents' rooms clean and comfortable
Collaborate with the interdisciplinary care team to develop and implement individualized care plans
Maintain accurate records and documentation
Support and promote a positive dining experience for residents
