Responsibilities
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor and document patient vital signs and health status using Electronic Health Record (EHR) systems
- Administer medications accurately following medical documentation protocols and HIPAA regulations
- Support patients with specialized care needs such as Alzheimer’s, dementia, memory care, hospice, and developmental disabilities
- Operate medical equipment including Hoyer lifts, catheters, and other assistive devices while ensuring safety standards are met
- Perform light housekeeping duties such as cleaning, laundry, meal preparation, and maintaining a safe environment
- Communicate effectively with healthcare teams regarding patient progress and medical records management
Skills
- Proven experience in home health or long-term care environments with a strong understanding of patient observation and medical terminology
- Skilled in assisting with ADLs, medication administration, catheter care, and hospice or dementia-specific care strategies
- Familiarity with EMR/EHR systems for accurate medical documentation and record keeping
- Knowledge of CPR certification and emergency response procedures to ensure patient safety at all times
- Ability to work with individuals with disabilities or developmental challenges in various settings including assisted living and nursing homes
- Excellent caregiving skills combined with compassion, patience, and strong communication abilities
- Capable of operating medical equipment such as Hoyer lifts while adhering to safety protocols
