Position Summary
At Signature Hospice, we believe caring for the whole person means caring for the body, mind, and spirit. While many organizations use the title Hospice Chaplain, we refer to this role as our Spiritual Care Coordinator because it reflects our commitment to providing compassionate, individualized support to every patient and family.
The Spiritual Care Coordinator serves as a trusted guide, offering emotional and spiritual care that honors each person's beliefs, values, traditions, and life experiences. Through presence, encouragement, prayer when requested, active listening, and meaningful conversation, this role helps patients and families find comfort, hope, peace, and dignity throughout their hospice journey.
Essential Responsibilities
- Provide compassionate spiritual and emotional support to hospice patients, families, and caregivers.
- Build meaningful relationships through routine patient visits in homes, nursing facilities, assisted living communities, and hospitals.
- Offer prayer, scripture, devotional readings, or spiritual conversation based on each patient's wishes and beliefs.
- Support patients and families through illness, grief, life review, forgiveness, legacy work, and end-of-life decision-making.
- Coordinate spiritual support with community clergy, pastors, and faith leaders when requested.
- Provide support and encouragement to caregivers before and after a patient's passing.
- Participate in interdisciplinary team meetings and collaborate with nurses, social workers, aides, volunteers, physicians, and leadership.
- Complete timely documentation in accordance with hospice regulations and organizational standards.
- Assist with memorial services, remembrance events, volunteer activities, and community education opportunities as appropriate.
Qualifications
- Ordination, ministry experience, chaplaincy training, theological education, or equivalent experience in spiritual care preferred.
- Demonstrated compassion, empathy, emotional maturity, and excellent listening skills.
- Ability to respectfully care for individuals of all faith traditions, spiritual backgrounds, and those with no religious affiliation.
- Strong communication and relationship-building abilities.
- Valid driver's license, reliable transportation, and ability to travel throughout the service area.

Pay: $27.50 - $29.00 per hour
