Join our dynamic healthcare team as a Physical Therapist specializing in skilled nursing facilities (SNF) in Winston-Salem, NC. Responsibilities
- Assess patients’ physical conditions by conducting thorough evaluations of mobility, strength, and functional abilities.
- Develop and implement individualized care plans aligned with clinical standards and patient needs.
- Provide hands-on physiotherapy treatments including therapeutic exercises, manual therapy, trigger point therapy, and modalities tailored to each patient’s condition.
- Document patient progress accurately using electronic health records (EHR) systems while adhering to clinical documentation standards.
- Collaborate with interdisciplinary teams to coordinate discharge planning and ensure seamless transitions across inpatient, outpatient, home health, or community care settings.
- Manage outpatient clinic and inpatient rehabilitation cases involving orthopedics, neurology, pediatrics, geriatrics, and other specialties.
- Educate patients and their families on physiotherapy techniques, injury prevention strategies, and home exercise programs to promote long-term wellness.
Qualifications
- Valid state licensure as a licensed Physical Therapist with practicing in a physical therapy role.
- Demonstrated experience working with geriatric patients in SNF or long-term care environments.
- Strong knowledge of physiology, anatomy, medical terminology, and clinical practices related to outpatient rehabilitation centers or inpatient hospital settings.
- Experience with discharge planning, care plan development, and working within EMR or EHR systems for documentation.
- Familiarity with acute care experience, inpatient hospital experience, home & community care settings, outpatient clinics, and outpatient rehabilitation centers.
- Skills in orthopedics, neurology, pediatrics, trigger point therapy, and geriatrics are highly desirable.
- Ability to communicate effectively with multidisciplinary teams while maintaining high standards of patient care and safety.

Pay: $2,900.00 - $3,000.00 per week
