Job Description:
As an LPN with our agency, you will be responsible for conducting supervisory health visits in client homes to evaluate caregiver performance, assess client condition, ensure compliance with care plans, and provide guidance to the care team.
Key Responsibilities:
- Perform supervisory visits in client homes in accordance with agency policies and state regulations
- Evaluate caregiver performance and provide coaching or corrective guidance as needed
- Assess client health status and overall satisfaction with services
- Ensure care plans are being followed appropriately and update documentation as needed
- Report any changes in client condition to the appropriate parties (RN, physician, family)
- Maintain accurate and timely documentation of all visits and assessments
- Serve as a liaison between clients, caregivers, and the clinical team
Qualifications:
- Current, active LPN license in the state of Georgia
- Minimum of 1 year of nursing experience, preferably in home care or a related field
- Strong assessment and documentation skills
- Ability to work independently and manage a flexible schedule
- Reliable transportation and valid driver’s license
