SAJE Strategic Solutions is seeking a skilled and independent Wound Care Clinician (NP, PA, MD, or DO) to provide mobile bedside wound care and G-tube management services across skilled nursing and long-term care facilities throughout California’s Central Valley.
This opportunity offers significant scheduling flexibility, allowing providers to work anywhere from 1 to 5 days per week, with compensation based on the selected schedule.

- 5 days/week: $130,000 annual salary
- 4 days/week: $104,000 annual salary
- 3 days/week: $78,000 annual salary
- 2 days/week: $52,000 annual salary
- 1 day/week: $26,000 annual salary
Schedule: 1–5 days per week
Territory: Sacramento, Stockton, Modesto & Turlock, CA
Setting: Skilled Nursing Facilities / Long-Term Care
Travel: Company travel vehicle may be available for high-volume, multi-site coverage
Position Overview
The Wound Care Clinician will provide specialized bedside wound care and G-tube management services to patients residing in skilled nursing and long-term care facilities throughout the Central Valley.
This position is ideal for an autonomous clinician who values schedule flexibility and is comfortable traveling between facilities while collaborating with interdisciplinary healthcare teams.
Essential Duties & Responsibilities
- Perform bedside clinical evaluations and provide treatment for acute and chronic wounds.
- Provide G-tube management and related clinical services.
- Assess patient needs and contribute to individualized clinical care plans.
- Collaborate with facility nursing leadership, interdisciplinary teams, and attending physicians.
- Complete accurate and timely electronic clinical documentation following each patient visit.
- Maintain compliance with practice protocols and applicable regulatory standards.
Qualifications
Licensure: Active, unencumbered California license as a Nurse Practitioner (NP), Physician Assistant (PA), Medical Doctor (MD), or Doctor of Osteopathic Medicine (DO).
DEA: Active DEA registration or eligibility to obtain registration.
Experience: Prior Skilled Nursing Facility (SNF) or wound care experience is strongly preferred. Hospital-based and acute-care clinicians are also encouraged to apply, with training on practice protocols provided.
Certification: Wound Care Certification such as CWCN, CWS, or WCC is preferred but not required.

You should be comfortable providing bedside care in post-acute environments, coordinating with facility staff and physicians, and managing patients across multiple locations.
