Experienced RN Manager – Private Duty Home Care Agency
Continuity Care Home Nurses, Inc., a busy and well-established private duty home care agency, is seeking an experienced Registered Nurse Manager with a BSN or MSN to join our leadership team.
This is an excellent opportunity for a hands-on RN leader who understands home care operations, is knowledgeable with Title 22 regulations, has strong clinical judgment, and wants to have a meaningful voice in patient care, staffing decisions, policies, procedures, and overall agency operations.
Position Summary
The RN Manager will help oversee the clinical and administrative operations of a private duty home care agency providing care in patients’ homes. Our field staff include RNs, LVNs, CNAs, CHHAs, and companions providing care from 4 to 24 hours per day.
We are looking for a positive, energetic, hands-on leader who can motivate staff, support quality patient care, promote regulatory compliance, and work collaboratively with the RN Owner, Administrator, Office Manager, QA/QI Nurse, staffing coordinators, and field employees.

Key Responsibilities
Responsibilities include, but are not limited to:
- Provide clinical leadership and support to field staff, including RNs, LVNs, CNAs, CHHAs, and companions.
- Conduct occasional field supervisory visits and inspect home charts with clients and nurses.
- Work closely with the existing QA/QI Nurse to support quality assurance, documentation compliance, regulatory compliance, and clinical standards.
- Train field staff on paperwork, documentation requirements, agency expectations, and applicable regulations.
- Lead weekly field staff orientation.
- Support and participate in office staff meetings with the Office Manager/Administrator.
- Assist with interviewing new field staff applicants and evaluating candidates for placement.
- Participate in the evaluation of field staff performance.
- Address performance concerns and complete corrective action/write-ups when needed.
- Support the RN Owner in maintaining high-quality care in patients’ homes.
- Assist with occasional marketing activities, including representing the agency at exhibit booths or community events with the RN Administrator.
- Help motivate staffing coordinators and other office staff while promoting teamwork, professionalism, accountability, and a positive work environment.
- Assist with policy and procedure development in a small agency environment where your input matters.
- Audit and prepare 485s as needed.

Qualifications
Required:
- Current California RN license in good standing.
- Minimum of three years of RN experience.
- Knowledge of Title 22 regulations and applicable home care/home health compliance requirements.
