We are looking for a skilled and dedicated individual who is able to support the access to equitable healthcare and mental health services to individuals living with HIV, and work successfully with people who have varying sexual and gender identities, economic and class status and potential substance use/misuse disorder. This individual must have the ability to provide strength-based services in a compassionate and respectful manner to clients who are most often experiencing homelessness, mental illness and need help navigating various public assistance in accessing complexed services through Ryan White providers and other related survival needs and advocacy services to our clients as needed.
CWP defines Care Case Manager for HIV and Mental Health" as a healthcare professional responsible for coordinating and managing comprehensive care for individuals living with HIV who also experience mental health challenges, ensuring they access necessary medical, psychosocial, and support services to optimize their overall well-being and treatment adherence.
Pay: $24.84 - $29.91 per hour
