Position Summary:
The Certified Nursing Assistant (CNA) provides compassionate, non-medical support to individuals in their homes and community settings. This role assists individuals with activities of daily living, personal care, mobility, nutrition, health monitoring, and community participation while promoting safety, independence, dignity, and overall well-being.
Position Scope:
The CNA works directly with individuals served under the supervision of the RN Case Manager and in accordance with individualized care plans. This position focuses on direct care and support services and does not carry supervisory, hiring, disciplinary, or clinical decision-making authority.
Key Responsibilities:
The CNA is responsible for delivering safe, respectful, and person-centered care that supports each individual's physical, emotional, and social well-being. Responsibilities include assisting with personal care, monitoring health and safety, supporting daily routines, documenting services provided, and communicating changes in condition to the appropriate care team members.
Secondary duties may include accompanying individuals to appointments, assisting with errands and community activities, supporting social and recreational engagement, maintaining household organization related to care needs, and providing additional support as assigned to meet individual service requirements.
Primary tasks include assisting individuals with activities of daily living, including bathing, grooming, dressing, toileting, personal hygiene, mobility, transfers, and positioning. Responsibilities include providing incontinence care, offering medication reminders, and observing for potential side effects or changes in condition. The CNA prepares and serves meals in accordance with dietary requirements, encourages proper nutrition and hydration, and performs light housekeeping duties such as laundry, dishwashing, dusting, and general cleaning to maintain a safe and comfortable environment. The role also involves providing companionship and emotional support, encouraging community engagement and social participation, assisting with transportation and errands when authorized, monitoring physical, behavioral, and emotional well-being, and maintaining accurate service documentation while promptly reporting concerns to supervisors and other appropriate care team members.
Regulatory Frameworks:
The CNA must comply with all applicable federal, Georgia state, and program-specific regulations, including HIPAA requirements, Georgia healthcare regulations, Medicaid waiver requirements where applicable, and all OneWell Health Care policies and procedures.
Confidentiality:
The CNA has access to protected health information (PHI) and sensitive personal information. Strict confidentiality must be maintained at all times in accordance with HIPAA, state privacy laws, and company policies. Unauthorized disclosure may result in disciplinary action up to and including termination.
Certifications:
- Active CNA certification or equivalent state-approved credential as required
- CPR and First Aid Certification
- Ability to safely assist with mobility and transfers
- Ability to follow individualized care plans and supervisor instructions
Education:
- High school diploma or GED required.
Experience:
Preferred:
- Prior experience as a Certified Nursing Assistant (CNA), Home Health Aide (HHA), Personal Care Assistant (PCA), caregiver, or related healthcare role
- Experience providing care in home and community-based settings
- Experience supporting individuals with disabilities, chronic conditions, or aging populations
Technical Skills:
- Ability to utilize electronic documentation systems and mobile applications
- Basic computer and smartphone proficiency
- Ability to accurately complete required documentation and reports
Languages:
English proficiency (spoken and written) is required. Bilingual skills are a plus based on individual and community needs.
Driver License:
Valid driver’s license may be required based on assignment. Must maintain an acceptable driving record and reliable transportation when driving is an essential job function.
Travel Requirement:
Occasional travel may be required.
