As a Geriatric Care Manager, you will develop trusted relationships with clients and families while coordinating many aspects of their care. You will serve as a central point of contact, helping clients navigate healthcare needs, coordinating services, communicating with providers, and advocating for their safety, independence, dignity, and overall quality of life.

What You’ll Do
As a Geriatric Care Manager, you will provide holistic, person-centered care and serve as a trusted resource for clients and their families. Responsibilities include:
- Assess Client Needs: Conduct comprehensive assessments to understand each client’s medical, social, emotional, environmental, and personal needs.
- Develop Personalized Care Plans: Create and implement individualized plans designed to promote safety, independence, dignity, and quality of life.
- Coordinate Care: Manage medical appointments, healthcare services, medications, in-home care, and communication among physicians and other members of the care team.
- Advocate for Clients: Ensure clients receive appropriate care, services, resources, and support while always keeping their best interests at the center of decision-making.
- Connect Clients with Resources: Identify and coordinate community resources, home care services, transportation, social programs, and other services that enhance daily living.
- Support Families: Provide compassionate guidance, education, and emotional support to families navigating the challenges and decisions that often accompany aging and caregiving.
- Monitor and Adapt: Continually evaluate each client’s needs and adjust care plans as health, safety, or personal circumstances change.
Above all, you will help older adults and their families navigate complex situations with greater confidence, dignity, independence, and peace of mind.
What We’re Looking For
The ideal candidate is a compassionate, resourceful RN who combines strong clinical judgment with exceptional communication and relationship-building skills.
Qualifications:
- Graduate of an accredited nursing program.
- Current Florida RN license or qualifying multistate RN license.
- Minimum of 2 years of nursing experience; care management experience is a plus.
- Previous home health experience is required.
- Valid Florida driver’s license and current automobile insurance.
- Strong organizational, critical-thinking, and problem-solving skills.
- Goal-oriented, proactive, and able to think creatively and adapt to changing client needs.
- Excellent verbal and written communication skills.
- Comfortable working with computers, including Windows and Microsoft Excel.
Work Schedule
- Monday through Friday, with flexibility to accommodate client needs.
- Minimal occasional on-call rotation throughout the year.
