Position Summary:
We are seeking a Registered Nurse (RN) to join our team on a PRN basis. In this vital role, the RN will oversee our Home Health Aide (HHA) program, ensuring the delivery of safe, compliant, and high-quality care. The RN will conduct initial assessments in client homes, develop individualized care plans, supervise and train HHAs, and collaborate with families and care teams to meet each client’s needs and perform skilled nursing services.This position is ideal for a self-motivated, clinically skilled RN who is passionate about community health, patient advocacy, and mentorship.
Key Responsibilities:
- Conduct comprehensive initial in-home assessments to evaluate client health, safety, and environmental needs.
- Develop and implement individualized care plans in collaboration with the Case Manager.
- Assign Home Health Aides to clients based on skills, compatibility, and care requirements.
- Perform routine supervisory visits to ensure care plan adherence and quality of service.
- Provide ongoing training, competency evaluations, and mentorship to HHAs.
- Respond to client emergencies and care issues as needed.
- Perform needed skilled nursing services for clients.
- Maintain accurate, timely, and professional clinical documentation.
- Monitor delivery of authorized services, ensuring they align with client needs and care goals.
- Promote a team-oriented, respectful environment through collaboration and leadership.
- Serve as a clinical resource to HHAs and care staff.
- Ensure compliance with all state/federal regulations and internal company policies.
Qualifications:
- Graduate of an accredited nursing program.
- Active, unrestricted RN license in the state of Indiana.
- Minimum of 2 years of nursing supervisory experience required.
- Minimum of 1 year of nursing supervisory or administrative experience required.
- Strong clinical judgment and the ability to work independently.
- Valid driver’s license, reliable transportation, and auto insurance.
- Excellent interpersonal, communication, and organizational skills.
- Flexible availability and willingness to travel to client homes.
Pay: $35.00 - $39.00 per hour
