Responsibilities:
Assessment and Documentation:
- Assess and thoroughly document individuals' level of functioning, health status, mental status, medical history, medications, medication compliance, and response to treatment.
Patient Education and Treatment Planning:
- Educate individuals receiving services about mental illness and substance use disorder.
- Participate actively in the development of treatment plans, crisis plans, and discharge plans.
Referral and Coordination of Care:
- Ensure that individuals who require a higher or different level of care are referred or placed appropriately.
Monitoring and Safety:
- Monitor individuals receiving services to ensure their health and safety.
- Carry out functions including crisis intervention, assessment of vital signs, referral for emergency medical treatment as necessary, and observation of individual response to treatment.
Specialized Nursing Functions:
- Follow detoxification protocols when applicable.
- Conduct nursing assessments and contribute to the overall treatment planning process.
- Engage in crisis and aftercare planning for individuals in the program.
Training and Professional Development:
- Attend and actively participate in meetings and training sessions as required.
- Maintain certification in all agency, state, and federal training requirements.
Compliance and Knowledge:
- Demonstrate knowledge of and comply with all agency policies and procedures.
- Adhere to state and federal statutes and regulations related to specific program areas.
Emergency Procedures:
- Demonstrate knowledge of emergency procedures and actively assist in crisis situations.
Skills:
- Proficient in medical terminology and documentation
- Knowledge of common medical procedures and techniques
- Ability to perform physical assessments and interpret patient data
- Strong communication skills for effective patient interaction and education
- Experience in providing primary care services to patients of all ages
