Dignity ADHC ECM Program is seeking a compassionate and motivated Bilingual Outreach Assistant to support member engagement, outreach activities, referral follow-up, and community outreach e9orts for the Enhanced Care Management (ECM) Program.The Outreach Assistant will work closely with the ECM Director and Care Management team to connect eligible members with ECM services, assist with outreach efforts, support enrollment activities, and help maintain relationships with community partners and referral sources.The ideal candidate is bilingual in English and Spanish, possesses excellent communication skills, enjoys helping others, and is comfortable speaking with members and community organizations.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Member Outreach & Engagement
- Conduct outbound outreach calls to prospective and assigned ECM members. Explain ECM program services and benefits to members and families. Schedule intake appointments and enrollment meetings. Follow up with members who are difficult to reach. Assist with member engagement and retention activities. Document all outreach attempts accurately and timely.
Referral Support
- Monitor and track incoming referrals. Assist with referral follow-up activities. Verify member contact information. Maintain outreach logs and referral tracking reports. Coordinate communication between members and ECM staff.
Community Outreach
- Assist with community outreach events and health fairs. Distribute marketing and educational materials. Support relationship-building efforts with community organizations and provider offices. Represent the organization professionally during outreach activities.
Administrative Support
- Maintain accurate records and documentation. Assist with data entry and reporting activities. Participate in staff meetings and training sessions. Support special projects as assigned. Maintain HIPAA compliance and confidentiality standards.
MINIMUM QUALIFICATIONS
Required:
- High School Diploma or GED. Fluent in English and Spanish (verbal and written). Valid California Driver’s License. Reliable transportation. Strong customer service and communication skills. Basic computer proficiency, including Microsoft Office. Ability to work independently and as part of a team.
Preferred:
- Experience in healthcare, social services, customer service, outreach, or community programs. Knowledge of Medi-Cal, CalAIM, or healthcare programs. Experience working with diverse populations.

- Strong verbal communication skills. Professional telephone etiquette. Ability to establish rapport with members and families. Organizational and time-management skills. Ability to maintain confidentiality. Dependable and self-motivated. Ability to multitask and prioritize assignments.
PHYSICAL REQUIREMENTS
- Ability to sit, stand, and walk for extended periods. Ability to travel locally for outreach activities and community events. Ability to occasionally lift up to 25 pounds.
WORK SCHEDULE
- Full-Time 37.5 hours per week Monday through Friday Occasional evening or community outreach events as needed
