JAG Healthcare Salem is currently seeking a compassionate, motivated, and team-oriented Physical Therapy Assistant (PTA) to join our rehabilitation team. In this role, you will assist in carrying out individualized therapy plans designed to help residents regain function, improve mobility, and maximize their independence under the direction of the supervising therapist and physician.

Responsibilities
(include but are not limited to):
- Assist in implementing individualized treatment plans established by the supervising Physical Therapist
- Carry out therapeutic exercises, activities, and interventions to improve residents’ functional abilities and independence
- Monitor resident progress and accurately document treatment sessions
- Communicate resident progress and concerns to the supervising therapist and interdisciplinary team
- Assist with screening, re-evaluation, discharge planning, and revisions to established plans of care as directed
- Educate residents and families on therapeutic exercises and adaptive techniques as appropriate
- Maintain compliance with all state, federal, and facility documentation and regulatory requirements
Skills & Qualifications
- Must be a Physical Therapy Assistant (PTA)
- Current Ohio license in good standing required
- Excellent communication, organizational, and time management skills
- Ability to work independently while collaborating effectively with an interdisciplinary team
- Long-term care or skilled nursing experience is preferred but not required
