As a Physical Therapist, you will assess, plan, and deliver individualized therapy programs designed to improve mobility, relieve pain, restore function, and enhance overall patient well-being. You’ll work closely with patients, families, and interdisciplinary care teams to provide exceptional home health care services.

Perform and document comprehensive patient evaluations and establish treatment plans

Develop and implement individualized therapy programs to restore physical function and mobility

Monitor patient progress and modify treatment plans to maximize outcomes

Educate patients and caregivers on exercises, safety, and continued care strategies

Maintain timely and accurate clinical documentation

Collaborate with physicians, nurses, and healthcare professionals to ensure continuity of care

Follow infection control and universal precaution guidelines

Support the agency’s mission, values, and patient-centered care philosophy

Graduate of an accredited Physical Therapy program
-
Current Physical Therapy license
-
CPR Certification
-
Valid driver’s license and auto insurance
-
Home Health experience preferred

HomeCare HomeBase experience preferred

Ability to stand, walk, bend, kneel, squat, and move throughout the day

Ability to lift, carry, push, or pull up to 100 lbs.
