Job Summary
We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our healthcare team. The LPN will provide essential nursing care to residents in a small 39 apartment assisted living community. This role requires a strong commitment to resident care, effective communication skills, and the ability to work collaboratively with both healthcare and non-healthcare professionals to ensure the highest standards of care.
Duties
- Administer medications and treatments as prescribed by physicians.
- Perform patient assessments and monitor vital signs.
- Assist with daily living activities for patients, ensuring comfort and dignity
- Collaborate with registered nurses and other healthcare team members to develop and implement patient care plans.
- Provide education and support to patients and their families regarding health management.
- Conduct triage assessments in emergency situations.
- Maintain compliance with HIPAA regulations and infection control protocols.
- Participate in the development of discharge planning for patients transitioning from acute care settings.
- Engage in ongoing professional development to enhance nursing skills and knowledge.
Qualifications
- Current LPN license in good standing.
- Experience in long-term care, or assisted living preferred
- Experience working with diverse populations, including those with Alzheimer’s
- disease.
- Ability to perform basic life support (BLS) and maintain current certification.
- Excellent communication skills and a compassionate approach to patient care.
