Job Summary
We are seeking compassionate and dedicated Home Care Workers to provide essential support and assistance to individuals in their homes. This role involves working closely with clients, including those from underserved communities and high-risk populations, to promote independence, safety, and well-being. The ideal candidate will possess strong communication skills, experience in social work or behavioral health, and the ability to handle diverse situations with professionalism and empathy. Responsibilities
- Assist clients with daily living activities such as prompting, reminders according to their individual hygiene plans
- Administer medications accurately following prescribed protocols
- Support behavioral health needs through behavior management techniques and crisis intervention when necessary
- Provide emotional support and companionship to clients, fostering a trusting relationship
- Conduct intake assessments and document client progress through data entry, data collection, and filing
- Support families by coordinating care plans and communicating effectively with other healthcare providers
- Assist with manual handling tasks ensuring safety for both clients and caregivers
- Support high-risk populations by implementing specialized care strategies tailored to individual needs
- Maintain accurate records of services provided, medication administration, and client interactions
Experience
Candidates should have relevant experience or skills including:
- Home & community care experience or related fields such as social work or behavioral health
- Supervising experience or working with families in caregiving roles
- Crisis intervention, conflict management, addiction counseling, or security background preferred
- Knowledge of medication administration, first aid, CPR, and manual handling techniques
- Experience working with individuals with mental health conditions or from underserved communities
- Strong communication skills for effective interaction with clients, families, and healthcare teams
