Registered Nurse (RN) Case Manager

BrightStar Care is seeking an experienced and dependable Registered Nurse Case Manager to join our Pensacola clinical team. This position is ideal for an RN who enjoys a combination of patient care, clinical oversight, nurse education, and staff development.
Position Summary
The RN Case Manager will support the clinical operations of the Pensacola office by completing infusion visits, non-skilled client assessments, nurse competency evaluations, and training for newly hired nurses. This role helps ensure clients receive safe, high-quality care and that field nurses are properly prepared to meet agency and regulatory standards.
Key Responsibilities
- Perform in-home infusion nursing visits.
- Complete initial, routine, and supervisory nursing assessments.
- Conduct non-skilled client assessments and caregiver supervisory visits.
- Complete orientation and clinical competency evaluations for newly hired nurses.
- Train, educate, and mentor new RNs and LPNs.
- Verify that field nurses demonstrate the required skills before beginning patient assignments.
- Review care plans and ensure services are being provided safely and appropriately.
- Communicate changes in client condition to physicians, families, and the clinical team.
- Maintain accurate and timely clinical documentation.
- Provide ongoing clinical support to field nurses and caregivers.
- Assist with quality assurance and regulatory compliance activities.
- Participate in the on-call rotation as needed.
- Travel throughout Escambia, Santa Rosa, Okaloosa, and Walton Counties to complete client visits, nurse competencies, supervisory visits, and new hire training.
Qualifications
- Current and unrestricted Florida Registered Nurse license.
- Minimum of two years of RN experience preferred.
- Home health, private duty nursing, case management, or infusion experience preferred.
- IV and infusion nursing experience strongly preferred.
- Strong clinical assessment and documentation skills.
- Excellent communication, organization, and time-management skills.
- Ability to work independently and manage multiple priorities.
- Current BLS or CPR certification.
- Valid driver's license, reliable transportation, and current automobile insurance.
Schedule
- Full-time
- Monday through Friday
- Daytime hours
- Occasional on-call responsibilities
- Travel required throughout Escambia, Santa Rosa, Okaloosa, and Walton Counties.
