We are looking to add a Registered Nurse/Case Manager to our care team to serve Boulder County and the surrounding communities. The RN provides direct care to adult and pediatric patients within the home setting to ensure safe recovery from illnesses and injuries. You will work closely with all members of our nursing and therapy team to ensure the best possible outcomes for patients. Patients needs include, but are not limited to:
- Wound Care
- Wound Vac
- Therapy IV
- Antibiotic therapy
- Medication management
- CNA supervision
Colorado RN or compact nursing license is required and prior home health RN experience is a plus.
Additionally this role is part of our on-call rotation.

Multiple days a week of availability is preferred.
Pay rates:
Visit rates: $53.90-$98/visit (**Note this is not per hour, is based on type of visit and dependent upon experience)
Hourly rate for meetings, case conference, training, orientation: $29.40-$34.30
