GuildCare, Lighthouse Guild’s innovative community-based Adult Day Health Care Program providing services to adults who are blind or visually impaired and have chronic health care needs is seeking a Part-Time Licensed Physical Therapist 10-14 hours per week for its Manhattan location.
Responsibilities:
- Evaluate, plan, develop, and implement physical therapy services as ordered.
- Assure that therapy personnel and registrants follow established policies and procedures at all times.
- Review physical therapy policies as requested, and participate in making recommendations for changes.
- Maintain a working relationship with registrant’s community physician.
- Review requests for physical therapy and physician's orders. Interview and evaluate registrant to determine type of treatment and schedule therapy.
- Participate in development and implementation of registrant care plans, registrant assessments and discharge planning, etc., as necessary.
- Comply with established safety precautions at all times.
- Complete necessary forms, reports, etc., and submit to the Program Director as required.
- Maintain treatment records and progress notes as required per GuildCare policy.
- Recommend to the Program Director the equipment and supply needs of the department.
- Assure that the therapy area is maintained in a clean and safe manner for registrant comfort and convenience by assuring that necessary equipment and supplies are maintained to perform such duties/services.
- Assure that all therapist notes are informative and descriptive of the care provided and of the registrant's response to the care.
- Complete service logs accurately and in a timely manner.
- Standing for long periods, walking, lifting/transferring registrants, using lifting equipment when required.
- Bending, stooping, squatting, twisting, reaching above and below shoulder level.
- Able to push/pull registrants in wheelchairs.
- Able to lift and carry equipment and/or supplies
15. Along with all other GuildCare staff, performs general assistance tasks on a regular basis as assigned by program director. These include: escorting registrants throughout the space, including to and from the restroom, assisting with transportation, serving meals, cleaning up from meals as needed, and filling in for absent coworkers.
16. Responsible for maintaining the confidentiality of registrant protected health information.
17. Responsible for following the LGI Workforce security policy; will access information systems and ePHI authorized by Clinical Authorization only.
18. Observes and implements the Registrants' Bill of Rights.
Qualifications:
- Master's degree with New York State license and certification is required.
- Experience in hospital/clinic/long term care setting and needs of visually impaired population, preferred.
