Experienced Family Nurse Practitioner or Physician AssistantPrimary Care & Transitional Care Management
East Valley Integrated Hospitalist Group is seeking an experienced and compassionate Family Nurse Practitioner (FNP) or Physician Assistant (PA) to join our team at a newly established outpatient office.
The selected provider will help manage our Primary Care and Transitional Care Management (TCM) service lines. Clinical responsibilities will be performed under the supervision and guidance of a supervising physician, in accordance with applicable laws, regulations, and organizational policies.
The group also has a dedicated Patient Navigator and Hospitalist Coordinator who will collaborate closely with the provider and provide operational and care-coordination support for Primary Care and TCM services.
Position Responsibilities
- Provide comprehensive primary care services to adult patients under physician supervision
- Perform new-patient evaluations, routine follow-up visits, annual wellness visits, and preventive care
- Evaluate and manage acute and chronic medical conditions
- Conduct post-discharge Transitional Care Management visits
- Review hospital discharge summaries, laboratory results, imaging, medications, and treatment plans
- Complete medication reconciliation and identify gaps in care
- Develop and implement individualized treatment plans in collaboration with the supervising physician
- Collaborate with the Patient Navigator to coordinate specialist referrals, home health services, therapy, medical equipment, transportation, and other community resources
- Work closely with the Hospitalist Coordinator to support timely post-discharge follow-up, obtain hospital records, and facilitate communication between the inpatient and outpatient care teams
- Coordinate care with hospitalists, supervising physicians, specialists, medical assistants, and other members of the multidisciplinary team
- Consult with the supervising physician regarding complex cases, changes in patient condition, and treatment decisions
- Educate patients and families regarding diagnoses, medications, treatment plans, and preventive care
- Maintain accurate, complete, and timely documentation in the electronic medical record
- Support the development and continued growth of the Primary Care and Transitional Care Management programs
Qualifications
- Graduate of an accredited Nurse Practitioner or Physician Assistant program
- Current and unrestricted Arizona APRN or Physician Assistant license, or eligibility to obtain licensure
- National board certification as an FNP or PA
- Current DEA registration, or eligibility to obtain one
- Current BLS certification
- Prior experience in primary care, internal medicine, hospital medicine, post-acute care, or transitional care management is required
- Strong clinical judgment, communication, and organizational skills
- Ability to work independently while collaborating closely with the supervising physician and multidisciplinary care team

Schedule
This is a full-time position with a consistent weekday schedule.
