Wyoming Range Rehab services in home therapies and we need a skilled professional Physical Therapist Assistant to join our team! We service Rock Springs, Green River, Bridger Valley, Evanston and beyond! Our primary need right now is in Sweetwater County (Rock Springs / Green River) but are open to all possibilities. Schedules are extremely flexible, choose your own hours, and help make a difference in patient's lives who are home bound and looking to get back to full independence in the home and community!
Duties
- Assist licensed physical therapists in developing and executing individualized care plans for patients of all ages, including pediatrics and geriatrics.
- Implement physical therapy treatments such as manual therapy, trigger point therapy, therapeutic exercises, and modalities to facilitate recovery.
- Operate medical equipment safely and effectively during therapy sessions, ensuring proper usage aligned with clinical standards.
- Document clinical assessments, treatment progress, and discharge planning accurately within electronic health records (EHR) systems following medical documentation standards.
- Communicate clearly with patients and caregivers to explain procedures, address concerns, and motivate engagement in therapy programs.
- Collaborate with multidisciplinary healthcare teams—including physicians, nurses, and occupational therapists—to optimize patient outcomes.
- Support discharge planning processes by providing detailed progress reports and recommendations for ongoing care or home exercises.
Experience
- Prior experience working in inpatient settings, skilled nursing facilities, outpatient clinics, or home health environments is highly preferred.
- Demonstrated knowledge of physiology, anatomy, orthopedics, and medical terminology to support effective treatment implementation.
- Skilled in clinical assessment documentation and maintaining compliance with healthcare standards for medical documentation.
- Hands-on experience with physical therapy techniques such as manual therapy and trigger point therapy is desirable.
- Familiarity with outpatient care, geriatrics, pediatrics, acute care settings, and client communication enhances your ability to deliver comprehensive care.
- Experience operating medical equipment safely while adhering to clinical protocols is essential.
- Strong communication skills for engaging with patients of diverse backgrounds and collaborating seamlessly within healthcare teams.
