Responsibilities
- Assist individuals with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary.
- Support medication administration in accordance with care plans and HIPAA regulations to ensure safety and compliance.
- Prepare nutritious meals tailored to dietary needs, assist with feeding, and maintain cleanliness in meal areas.
- Implement individualized care plans by providing personalized support that promotes independence and respects each person’s preferences.
- Maintain a safe environment through regular cleaning, sanitation, and organization of living spaces in group homes or home settings.
- Document care activities accurately, including patient observations, progress notes, and incident reports to ensure continuity of care.
- Be familiar with Brittco, reading ISP's, and doing daily goals.
Qualifications
- Prior experience working with individuals with developmental disabilities, autism, dementia (including Alzheimer’s and memory care), or in assisted living/nursing home settings is highly preferred.
- Knowledge of first aid, CPR certification, and basic patient monitoring techniques is essential for ensuring safety.
- Familiarity with HIPAA regulations and confidentiality standards related to patient care documentation.
- Ability to perform heavy lifting safely when assisting residents with mobility or transfers using equipment like Hoyer lifts.
- Strong communication skills to effectively collaborate with team members, residents, families, and healthcare providers.
- Demonstrated patience, compassion, and a genuine desire to support individuals with diverse needs in their daily routines.
- Valid certifications in caregiving or home health care are a plus; ongoing training in dementia care or hospice support is beneficial.
