Job Summary
As a Home Health Registered Nurse, you will provide skilled nursing care to patients in their homes, including assessments, care planning, medication administration, infusion therapy, wound care, and patient education. Working independently while collaborating with physicians and other healthcare professionals, you'll help improve patient outcomes and support individuals in maintaining their independence.
Responsibilities
- Conduct comprehensive in-home patient assessments.
- Develop, implement, and update individualized care plans.
- Administer medications, IV therapies, injections, and infusion treatments.
- Perform wound care, dressing changes, and monitor patient progress.
- Monitor vital signs and respond to changes in patient condition.
- Educate patients and caregivers on medications, disease management, and home care.
- Coordinate care with physicians, pharmacies, therapists, and other providers.
- Supervise Home Health Aides and complete required supervisory visits.
- Maintain accurate and timely electronic documentation.
- Follow agency policies and all state and federal regulations.
- Participate in an on-call rotation as scheduled.
Qualifications
- Graduate of an accredited school of nursing.
- Current, unrestricted Registered Nurse (RN) license in North Carolina.
- Current CPR certification.
- Minimum of 1 year of nursing experience (home health and infusion experience preferred).
- Experience with IV therapy, vascular access, and wound care is a plus.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and patient education abilities.
- Comfortable working independently in the home setting.
- Valid driver's license, reliable transportation, and current auto insurance.
- Must successfully pass a background check.
Physical Requirements
- Ability to lift up to 50 pounds.
- Ability to stand, walk, bend, and assist patients throughout the workday.
- Ability to travel throughout the service area for patient visits.
