Position Summary
The admissions Coordinator is responsible for managing the admissions process for prospective residents and patients, serving as a liaison between referral sources, families, and the nursing home team. This role ensures a smooth transition into the facility while maintaining high occupancy and excellent customer service.
Key Responsibilities
Coordinate and manage resident admissions from hospitals, rehabilitation centers, and community referrals.
Communicate with prospective residents, families, and referral sources regarding services and admission requirements.
Verify insurance benefits and obtain necessary documentation.
Conduct facility tours and answer questions about care and services.
Collaborate with nursing, social services, and administration to ensure a seamless admission process.
Maintain accurate admission records and census data.
Develop and maintain positive relationships with hospitals, physicians, case managers, and community partners.
Assist with marketing and outreach efforts to increase referrals and occupancy.
Qualifications
High school diploma required; associate or bachelor's degree preferred.
Previous healthcare, admissions, customer service, or long-term care experience preferred.
Knowledge of Medicare, Medicaid, and insurance processes preferred.
Strong communication, organizational, and interpersonal skills.
Proficient in Microsoft Office and electronic medical records systems.
